Scheduling an Appointment Patients may schedule appointments during regular office hours between 8:00 AM and 5:00 PM by contacting the front office at 480 502-5533.
What to Bring to Your Appointment

Insurance Card
-
Copay or deductible, either cash or credit card. Your copay or deductible can often be found on your insurance card.

A list of current prescription medications. Bringing the actual prescription bottles are preferred so that Dr. Betz can accurately verify all of the information regarding dosing and frequency.

Name, address and phone numbers of previous physicians.

Bringing previous medical records and labs will help greatly for our new patient appointments.
Annual blood work for new patients and established patient for annual exams:

If you anticipate annual blood work to be done, please consider showing up to your appointment "fasting" for 8 hours prior, so that if medically indicated we can draw fasting blood work (usually this is for cholesterol/lipid screening and diabetes screening).

You can have water, black coffee or tea (no cream or sugar), no food or other beverage.

You can usually take all your routine medications (call if you are a diabetic or if you have questions)
Canceling an Appointment We appreciate being given a 24 hour notice of cancellation and will work to get you rescheduled at your earliest convenience.
Refills Please contact your pharmacy first when you need a refill. The pharmacy will fax our office the appropriate forms to complete the process. The doctor may ask that you schedule an appointment if it has been a while since your last visit so that he can accurately monitor your health. Please allow 3 business days for refill requests to be completed.
Contacting Physician After Hours Dr. Betz is typically available after hours for emergencies. On rare occasions you may be directed to the ED or Urgent Care. To reach the doctor after hours simply call the office and follow the prompts on the message. Dr. Betz will not be reviewing routine test results or refilling routine Rxs, pain medications or antibiotics outside of office hours. If you are having a significant medical emergency, such as symptoms of heart attack or stroke, please call 911.
